This is a three-axis XYZ manipulator / positioner manufactured by Huntington Mechanical Laboratories, Inc. The model number is MPM-275, serial number 87036. It is missing the two motors for the X and Y motions. Equipped with CF 275 flange. No rotary motions.

X – Y Motion – Plus or minus 0.500 inches.
Z Motion – 1.500 inches.
Z shaft pitch – 0.125 inches

Does not include bellows – this is just the mechanical translation portion with the Z-axis motor.
